Admission Check list

The following documents are needed to complete an application file:

  • Completed ISA application form
  • 100.00 Awg. application fee for Grades 1-12
  • 50.00 Awg. application fee for Montessori
  • 2 current photographs of student
  • Copy of the first page of student's passport
  • Copy of student’s birth certificate
  • 3 years of prior school records (including up-to-date grades for the current school year)
  • If available: standardized test results - (Iowa, California Achievement Tests, Stanford, Terra Nova, etc.)
  • If available: special education records, including IEP and any individual test results.
  • ISA medical information form
  • Copy of immunization record

Documents needed after Acceptance.

  • ISA student and parent handbook form (signed during first week of school)
  • Annual field-trip permission form (signed during first week of school)
  • Computer usage policy form (signed during first week of school)
  • Photo consent form
  • ISA Student Transportation Contract
