The following Documents are required to make a complete Application file:
- Completed ISA Application form
- Submit 2009-10 Application with 100 AFL fee (required) and 50 AFL fee for Montessori
- 2 current photographs of student
- Copy of the first page of student’s passport
- Copy of the birth certificate
- 3 years of prior school records (including up to date grades for the current school year)
- Standardize test results- if available (Iowa’s, California Achievement Tests, Stanford’s, Terra Nova’s, etc.)
- Special education records, including IEP and test results
Documents needed after Acceptance but before a student can begin classes
- ISA medical information form
- Copy of Immunization record
- ISA student and parent handbook form (signed)
- Annual field trip permission form (signed)
- Computer usage policy (signed)
